




Self Direction is New York State’s current self-determination program. Self-direction empowers people to design supports based on their unique strengths and needs so they can live the life they want. Individuals and families work together with a team to customize services such as Community Habilitation, Respite and more.
Individuals interested in Self-directing their services should *Reside in Cortland, Tioga and Tompkins Counties or parts of Broome, Chemung, Stueben, Seneca, Schyler, Cayuga, and Onondaga counties *Have OPWDD approval for services, *Be enrolled in Care Management services, and *Have HCBS Waiver Services Authorization
Individuals may be referred for services through their Care Manager. Individuals who do not yet have OPWDD approval may contact their local Service Access Coordinator for assistance.

Unseen offers an unfiltered, honest glimpse into the lives of parent caregivers of children and adults with disabilities or complex medical needs. Many caregivers face exhaustion and isolation, creating a public health crisis that affects us all. This documentary aims to raise awareness and spark change for millions of families. After the screening, a facilitated discussion will be led by Susan Thomas, LCSW-R, and Sarah Elliot, LMSW. Light refreshments will be provided.
