Community Habilitation

Community Habilitation services are person-driven skill building activities.  Goals and desired outcomes are developed by the person receiving services with support from a team of their choosing, such as family, and staff.  These services may be provided at home, if the person does not

live within a certified setting, or in the community to build skills such as: 

  • Money management
  • Independent living skills
  • Food preparation
  • Personal care
  • Independent travel
  • Banking
  • Relationship development
  • Volunteer work
  • Social skills
  • Leisure skills
  • Community exploration

Eligibility: Individuals interested in Community Habilitation services through Racker should:

  • Have a need and desire to build their skills
  • Have OPWDD eligibility
  • Have HCBS Waiver Services Authorization
  • Be approved for Community Habilitation Services through OPWDD

How to access: Individuals may be referred for services through their Care Manager. Individuals that do not yet have OPWDD approval may contact their choice of Care Coordination Organizations (CCO) or local Service Access Coordinator for assistance.
